Before you use it

  • Write one fact per line — each becomes a numbered "That…" paragraph. Keep facts specific (dates, document numbers).
  • Do not sign in advance: sign only before the Notary Public / Oath Commissioner / Magistrate, with original ID.
  • Ask the receiving office whether stamp paper is needed and of what value, or whether a self-declaration is enough.
  • For court cases, use the court's own format — ask an advocate.

Notes before use (attestation and stamp rules vary by State):

  • Sign this affidavit only in the presence of a Notary Public, Oath Commissioner or Executive / Judicial Magistrate, who will administer the oath and attest it. Carry an original photo ID.
  • Print it on non-judicial stamp paper / e-stamp of the value required in your State, or on plain paper if the receiving office accepts that (many offices now accept self-declarations instead of affidavits — ask them first).
  • Making a false statement on oath is a punishable offence (Bharatiya Nyaya Sanhita, 2023, Sections 227–229). Write only what you know to be true.
  • Affidavits filed in a court case follow that court's own format and rules; this is a general-purpose format.
